What we found in 29 US practice audits

Before taking on clients here, we audited 29 dental, orthodontic, and dermatology practices across the United States, using only publicly visible information, to understand where local visibility actually breaks down. The pattern was consistent enough to be worth publishing.

FINDING 01

Empty services sections

Nearly every profile had an incomplete or blank services section. It's one of the strongest signals Google uses to match a practice to a treatment search, it takes an afternoon to fill in properly, and it's almost always left empty.

FINDING 02

Absent from AI answers

Most practices didn't appear in ChatGPT or Gemini responses for their own primary treatment in their own city. In each case, two or three local competitors were named instead.

FINDING 03

Category and proximity mismatches

Primary category and service-area setup were frequently working against the practice, keeping it out of the map pack for searches happening inside its own neighborhood.

FINDING 04

Zero brand mentions

Most practices were never mentioned anywhere on the web outside their own website. Language models build confidence in a business from how consistently it is named across independent sources, not from backlinks alone. A practice nobody writes about gives AI no reason to name it.

FINDING 05

Practitioner citation drift

The doctor's own name appeared with different spellings, credentials, or addresses across Healthgrades, Zocdoc, the state board, and the NPI registry. Search engines treat the practitioner as an entity separate from the practice. When one is unclear, both get weaker.

FINDING 06

No entity authority

Almost none had structured data connecting the practice to its named doctors, and none held a knowledge panel of its own. Without that link a clinic is a string of text rather than a known thing an AI can confidently recommend.
